Medical errors are one of the leading causes of death and
injury in the U.S. In fact more people die annually from medical
errors than automobile accidents, breast cancer or even AIDS.
Unfortunately, there is a common misperception that medical
errors only result from problems with surgery or medication,
such as operating on the wrong leg or an incorrect dosage
